Submit your Unclaimed Funds Claim Form
The completed form can be mailed or delivered to the Auditor's Office. Only original documents can be submitted - photocopies, faxes, and PDFs are not accepted.

Proof of Claim Requirements
1. Notarization
Each unclaimed payment requires a separate notarized form.

2. Proof of Claim

Include the following documents with your form:

For Individuals:
​

Photocopy of your Social Security card

Photocopy of your Driver’s License or State ID

Original check(s), if available

For Corporations:
​

Copy of the front page of the tax return or document with Federal ID Number

Proof of authority* for the person signing the form (e.g., corporate resolution, affidavit from a senior officer)

Verification of position* (e.g., board minutes, SEC filings, annual reports)​
*Business cards or letterhead are not acceptable as proof.
